The forms of the unified state exam are approved by Ro-sobr-nad-zo-rum. There are 4 types of Unified State Examination forms:

• registration form (including oral exam registration form)
• report form No. 1 for recording the results of completing assignments in a short form
• report form No. 2 for recording the results of completing tasks in expanded form
• complete form from the authorities No. 2 for recording the results of completing tasks in expanded form

ATTENTION!

• An incorrectly completed Unified State Examination form may cause test failure.
• Unified State Examination forms damaged during registration cannot be replaced.
• A new set of examination materials is issued only if a Unified State Examination participant discovers that the examination materials are defective or incomplete.

Therefore, when preparing for exams, it is important to study the forms carefully. Below are sample USE forms. Practicing filling them out will help you save time on the exam and avoid embarrassing mistakes due to incorrectly recording answers.

General requirements for filling out forms

• The data must be filled out with a gel or capillary pen with black ink in capital block letters according to the sample. This measure will help avoid scanning errors.
• Even in rough drafts, do not use other colored ink or pencil, or “putty” for corrections.
• Fill in each field starting from the first position.
• When filling out the fields in all forms, use the spelling of symbols at the top of form No. 1 as a sample.
• If you do not have information to fill out the field, leave it blank.
• Do not make any notes or notes on the answer forms that could identify you.
• Forbidden make any entries or notes not related to these forms in the fields of the forms, outside these fields and in the fields filled in typographically.

Registration form

The registration form is intended for entering in-di-vidual data of the graduate. At the top of the sheet there is a vertical and horizontal barcode with its digital number.

• code of the educational institution where the graduate of the current year studied (for graduates of previous years or enrolling in a college/university - the code of the educational institution in which the participant received a pass to the Unified State Examination);

• code of the Unified State Examination point;
• number of audience at the point where the Unified State Examination is conducted;
• yes, conducting the Unified State Examination;
• pre-meta code;
• name of the subject;
• for service use.

Codes Unified State Exam subjects:

In the middle part The registration form indicates information about the Unified State Examination participant:

• surname
• on behalf of (if available)
• series and number of do-cumen-ta, accrediting-believing-personality

Also, the middle part of the form contains brief instructions on how to conduct the Unified State Exam and completing forms. op-re-division of the whole-nos-ti in-di-vidu-al-no-go comp-lek-ta of the Unified State Examination participant, for the sub-pi-si-participation for the Unified State Exam and for service use.

The Unified State Examination participant fills out the upper and middle parts of the form (except for the field for official use) . The signature of the exam participant is placed strictly inside the “window”.

In the lower part the registration form is filled out in the correct order by the official organizator in the audience in the event -yah, if the participant is removed from the examination due to a violation of the Unified State Examination procedure or has not completed the examination for a good reason. Answer form No. 1

The Unified State Examination in all subjects consists of two parts. All tasks in each CMM version are presented in continuous numbering mode.

Report form No. 1 is intended for recording the results of completing tasks in a short form.

Rules for filling out Unified State Exam forms

Unified State Examination participants complete examination papers on Unified State Examination forms, the forms and description of the rules for filling them out are given below. When filling out forms, you must strictly follow these rules, since the information entered in the forms is scanned and processed using special hardware and software.

When filling out answer forms No. 2, you must indicate “look on the back” if you turn to the back side.

If there is not enough space to write down answers to tasks with a detailed answer on answer form No. 2 (including the back side of the form), the organizer in the audience, at the request of the Unified State Exam participant, issues an additional answer form No. 2. In this case, the organizer indicates the number of the additional answer form No. 2 in the previous answer form No. 2. Additional answer forms No. 2 are not accepted for assessment if at least one of the sides of the previous answer forms No. 2 is not completed.

Basic rules for filling out Unified State Exam forms

All Unified State Exam forms are filled out with a black gel or capillary pen. The mark symbol (“cross”) in the fields of the registration form should not be too thick. If the pen leaves a too thick line, then instead of a cross in the field you need to draw only one diagonal of the square (any).

The Unified State Exam participant must depict each number and letter in all filled-in fields of the registration form, answer form No. 1 and the top of answer form No. 2, carefully copying the sample of its spelling from the line with samples of character writing located at the top of the registration form and answer form No. 1 . Careless writing of symbols can lead to the fact that the symbol may be recognized incorrectly during automated processing.

Each field in the forms is filled out, starting from the first position (including the field for entering the last name, first name and patronymic of the Unified State Exam participant). If a USE participant does not have information to fill out a specific field, he must leave this field empty (do not make dashes).

When writing down answers, you must strictly follow the instructions for completing the work (for a group of tasks, individual tasks) specified in the CMM. On answer forms No. 1 and No. 2, as well as on the additional answer form No. 2, there should be no marks containing information about the identity of the Unified State Exam participant.

BASIC RULES FOR COMPLETING USE FORMS

All Unified State Exam forms are filled out with a black gel or capillary pen.

In the middle part The registration form contains the following information about the exam participant: (filled out in accordance with the examples of writing letters and numbers):

• last name, first name, patronymic (if available);
• series and number of the identity document.
• In the middle part of the registration form there are:
• a brief reminder about the procedure for conducting the Unified State Exam;
• brief instructions for determining the integrity and correctness of the printing of the individual set of the exam participant;
• field for the exam participant’s signature confirming familiarization with the procedure for conducting the Unified State Exam.

In the lower part The registration form contains fields for official use (“Office mark”, “Reserve-2”, “Reserve-3”), fields filled in by the responsible organizer in the PPE classroom in cases where an exam participant is removed from the exam due to a violation of the Unified State Examination Procedure or did not complete the exam for objective reasons, as well as a field for the signature of the responsible organizer. The fields “Service mark”, “Reserve-2”, “Reserve-3” are not filled in.

In the lower part One-sided answer form No. 1 provides:

• fields for replacing erroneous answers to short answer tasks. The maximum number of fields for replacing erroneous answers is 6, the maximum number of characters in one answer is 17;
• the field “Number of completed fields “Replacement of erroneous answers”, filled in by the responsible organizer in the audience, and the field for the signature of the responsible organizer.

Answer form No. 2 is a one-sided machine-readable form and consists of two parts - upper and lower.

At the top Answer form No. 2 (sheet 1 and sheet 2) contains special fields that indicate the form and year of the examination (the words “Unified State Exam - 2020”), as well as the name of answer form No. 2 indicating the serial number of the sheet. The specified fields are filled in typographically.

Also at the top of answer form No. 2 are:

Hello almost everyone! The Unified State Exam is just around the corner - the main stage. Many are really ready, but many are really not. But today’s topic will be important for everyone. The fact is that incorrectly filled out USE 2016 forms are a sure failure in the exam. It may seem to you that everything is obvious: you take the form and fill it out. In fact, there are points that are not obvious (at least in history and social studies) that are really better to keep in mind in advance.

The reason for writing this note was a question from one of my students. For some reason, nothing like this was explained to them at school. So let's get started. I’ll say right away that you can find general rules for filling out forms at the very end of this note.

How to fill out the Unified State Exam 2016 forms in history

Before you will have small instructions, which will look something like this:

And now the fun begins. At the very beginning, it may not be said exactly how to write down the names of the rulers of phrases and concepts. But the sample shows that, for example, the name of the ruler is written together, that is, WITHOUT SPACES.

After comparing different tasks, I made the following non-obvious conclusions and summarized them into the following recommendations:

Firstly, if the task says to write the name of the ruler, then write his name and serial number. For example:

NICHOLAYPERVIY

ALEXANDER II

PETER THE FIRST, etc.

Secondly, if the task requires you to write the name of the commander, his first and last name are again written together. For example:

ALEXANDER NEVSKIY

BARCLAYDETOLLY

MIKHAILKUTUZOV

Third, Keep track of what exactly you need to write in the assignment. If you need to write a last name, write only the last name; if you need to write a first name, write the first name. If you come across monosyllabic names, such as: KUCHUM, BATY, MAMAI - consider yourself lucky. If you come across the names of Russian commanders and you need to write the name, but the name is complex, then write the first and last names together, as shown in paragraph 2. If you need to indicate the name of the Grand Duke, write the first and patronymic, again CONTEMPORARLY. For example:

MIKHAILALEXANDROVICH

But such tasks are rare. For example, one of my students had an assignment in trial Unified State Exam write the name of the last Russian emperor. She wrote Mikhail Romanov (it is clear that she wrote MIKHAILROMANOV), but the correct word was MIKHAILALEXANDROVICH.

As a result, her score was reduced. Why is that? I would venture to guess that this happened because, according to the logic of the test compilers, there were at least two Romanov Mikhails: the first ruled in the 17th century and was the first of the Romanov dynasty, and the second ruled for a couple of days at the very end. Because of this, when indicating the answer “MIKHAILROMANOV”, it is not clear which one.

Therefore, if you come across approximately the same task, write your first and middle name. And if there is enough space and a last name. If something doesn’t count for you, feel free to file an appeal - this is a machine error and the fault of the test writers, but not yours!

Fourthly, You need to write all phrases, concepts and terms UNITEDLY. For example:
